How is home health care provided?

Home health care is provided by a team of health care professionals collaborating to ensure that a patient’s medical and daily needs are met safely and appropriately. After the doctor has given orders, health care professionals/caregivers can come to your residence to provide care according to the customized plan of care designed for you or your loved one’s needs.

How do you see health care changing over the next 5 to 10 years?

Heath care has been trending, and continues to trend towards outpatient care. Cost-effectiveness as well as patient-centered care models all point towards keeping people in the community as the most efficient way to achieve the best possible outcomes. Over time the demand for Doctors, nurses, aides, therapists and other health care providers functioning in the home setting will continue to grow. It is important for consumers and providers alike to remain well-informed of the home health services available.

Can a home care program help improve my health and wellness?

As the saying goes “There’s no place like home.” Home care gives you the freedom to continue doing things you love doing in your home, while receiving assistance with activities of daily living and managing your health. You can stay near family and neighbors, enjoy your garden, move around your own back yard, or stay close to your pet. The familiar surroundings provide a secured feeling, reduces stress and gives way to improved health and quality of life.

How can I exercise my rights as a patient?

The Department of Health outlines many rights in their Patient Bill of Rights. Perhaps one of the most important rights is the right to make informed decisions about your health and the care you choose (or refuse) to receive. Home Care Grid serves to help you, the consumer, get informed so that you can make the best possible decisions for yourself and your loved ones
